The harness along with a Karabiner, and Scaffold hook is a really low cost (about £30) and strongly recommended solution to ladder safety. Of course ladder needs to be secured - I use a couple of eyelets screwed into the wall with a length of rope. Can’t recommend enough to keep you safe.
